    Reduction in cardiovascular risk by sodium-bicarbonated mineral water in moderately hypercholesterolemic young adults.

    Effects of drinking a sodium bicarbonated mineral water on cardiovascular risk in young men and women with moderate cardiovascular risk were studied. Eighteen young volunteers, total cholesterol levels >5.2 mmol/L without any disease participated. The study consisted in two 8-week intervention periods. Subjects consumed, as a supplement of their usual diet, 1 L/d of a control low mineral water followed by 1 L/d of the bicarbonated mineral water (mmol/L: sodium, 48; bicarbonate, 35; and chloride, 17). Determinations were performed at the end of the control water period and weeks 4 and 8 of the bicarbonated water period. Body weight, BMI, blood pressure, dietary intake, total-cholesterol, LDL-cholesterol, HDL-cholesterol, Apo A-I, Apo B, triacylgycerols, glucose, insulin, adiponectin, high sensitivity-C reactive protein (hs-CRP), soluble adhesion molecules (sICAM and sVCAM), sodium and chloride urinary excretion, and urine pH were measured. Dietary intake, body weight and BMI showed no significant variations. Systolic blood pressure decreased significantly after 4 weeks of bicarbonated water consumption without significant differences between the weeks 4 and 8. Significant reductions were observed after bicarbonated water consumption of total cholesterol (by 6.3%, p=0.012), LDL-cholesterol (by 10% p=0.001), total/HDL-cholesterol (p=0.004), LDL/HDL-cholesterol (p=0.001), and Apo B (p=0.017). Serum triacylglycerols, Apo A-I, sICAM-1, sVCAM-1 and hs-CRP levels did not change. Serum glucose values tended to decrease during the bicarbonated water intervention (p=0.056) but insulin levels did not vary. This sodium bicarbonated mineral water improves lipid profile in moderately hypercholesterolemic young men and women and could therefore be applied in dietary interventions to reduce cardiovascular risk

    SIRT7 mediates L1 elements transcriptional repression and their association with the nuclear lamina

    Altres ajuts: La Marató de TV3.Long interspersed elements-1 (LINE-1, L1) are retrotransposons that hold the capacity of self-propagation in the genome with potential mutagenic outcomes. How somatic cells restrict L1 activity and how this process becomes dysfunctional during aging and in cancer cells is poorly understood. L1s are enriched at lamin-associated domains, heterochromatic regions of the nuclear periphery. Whether this association is necessary for their repression has been elusive. Here we show that the sirtuin family member SIRT7 participates in the epigenetic transcriptional repression of L1 genome-wide in both mouse and human cells. SIRT7 depletion leads to increased L1 expression and retrotransposition. Mechanistically, we identify a novel interplay between SIRT7 and Lamin A/C in L1 repression. Our results demonstrate that SIRT7-mediated H3K18 deacetylation regulates L1 expression and promotes L1 association with elements of the nuclear lamina. The failure of such activity might contribute to the observed genome instability and compromised viability in SIRT7 knockout mice. Overall, our results reveal a novel function of SIRT7 on chromatin organization by mediating the anchoring of L1 to the nuclear envelope, and a new functional link of the nuclear lamina with transcriptional repression

    Design and evaluation of a treatment programme for Spanish adolescents with overweight and obesity. The EVASYON Study

    Background The prevalence of overweight and obesity (OW/OB) among adolescents worldwide has increased since the 60 s. Spain has reached one of the highest OW/OB prevalence rates among adolescents from European countries. The aim of this methodological paper is to describe the design and evaluation in the EVASYON study (Development, implementation and evaluation of the efficacy of a therapeutic programme for adolescents with OW/OB: integral education on nutrition and physical activity). Methods/Design The EVASYON was planned by a multidisciplinary team to treat OW/OB in Spanish adolescents. The EVASYON is a multi-centre study conducted in 5 hospitals in 5 Spanish cities (Granada, Madrid, Pamplona, Santander and Zaragoza) and two hundred and four OW/OB Spanish adolescents were recruited for this intervention. The treatment was implemented for approximately one-year follow-up. The adolescents were treated in groups of a maximum of 10 subjects; each group had 20 visits during the treatment period in two phases: intensive during the first 2 months (1st to 9th visits), and extensive during the last 11 months (10th to 20th visits). In order to assess the efficacy of the treatment, 8 dimensions were measured: diet; physical activity and fitness; eating behaviour; body composition; haematological profile; metabolic profile; minerals and vitamins; immuno-inflammatory markers. Moreover, genetic polymorphisms were also determined. Discussion The treatment programme developed in the EVASYON study was designed as a national pilot study to be implemented as an effective treatment for adolescents with OW/OB into the Spanish Health Care Service

    Diet quality index as a predictor of treatment efficacy in overweight and obese adolescents: The EVASYON study

    Background & aim: A diet quality index (DQI) is a tool that provides an overall score of an individual''s dietary intake when assessing compliance with food-based dietary guidelines. A number of DQIs have emerged, albeit their associations with health-related outcomes are debated. The aim of the present study was to assess whether adherence to dietary intervention, and the overall quality of the diet, can predict body composition changes. Methods: To this purpose, overweight/obese adolescents (n = 117, aged: 13–16 years; 51 males, 66 females) were recruited into a multi-component (diet, physical activity and psychological support) family-based group treatment programme. We measured the adolescents’ compliance and body composition at baseline and after 2 months (intensive phase) and 13 months (extensive phase) of follow-up. Also, at baseline, after 6 months, and at the end of follow-up we calculated the DQI. Results: Global compliance with the dietary intervention was 37.4% during the intensive phase, and 14.3% during the extensive phase. Physical activity compliance was 94.1% at 2-months and 34.7% at 13months and psychological support compliance were growing over the intervention period (10.3% intensive phase and 45.3% during extensive phase). Adolescents complying with the meal frequency criteria at the end of the extensive phase had greater reductions in FMI z-scores than those did not complying (Cohen''s d = 0.53). A statistically significant association was observed with the diet quality index. DQI-A variation explained 98.1% of BMI z-score changes and 95.1% of FMI changes. Conclusions: We conclude that assessment of changes in diet quality could be a useful tool in predicting body composition changes in obese adolescents involved in a diet and physical activity intervention programme backed-up by psychological and family support

    The comparative responsiveness of Hospital Universitario Princesa Index and other composite indices for assessing rheumatoid arthritis activity

    Objective To evaluate the responsiveness in terms of correlation of the Hospital Universitario La Princesa Index (HUPI) comparatively to the traditional composite indices used to assess disease activity in rheumatoid arthritis (RA), and to compare the performance of HUPI-based response criteria with that of the EULAR response criteria. Methods Secondary data analysis from the following studies: ACT-RAY (clinical trial), PROAR (early RA cohort) and EMECAR (pre-biologic era long term RA cohort). Responsiveness was evaluated by: 1) comparing change from baseline (Delta) of HUPI with Delta in other scores by calculating correlation coefficients; 2) calculating standardised effect sizes. The accuracy of response by HUPI and by EULAR criteria was analyzed using linear regressions in which the dependent variable was change in global assessment by physician (Delta GDA-Phy). Results Delta HUPI correlation with change in all other indices ranged from 0.387 to 0.791); HUPI's standardized effect size was larger than those from the other indices in each database used. In ACT-RAY, depending on visit, between 65 and 80% of patients were equally classified by HUPI and EULAR response criteria. However, HUPI criteria were slightly more stringent, with higher percentage of patients classified as non-responder, especially at early visits. HUPI response criteria showed a slightly higher accuracy than EULAR response criteria when using Delta GDA-Phy as gold standard. Conclusion HUPI shows good responsiveness in terms of correlation in each studied scenario (clinical trial, early RA cohort, and established RA cohort).     Copper bioavailability from breakfasts containing tea : influence of the addition of milk

    The influence of drinking tea on copper bioavailability is unclear, particularly when tea is consumed with food. A breakfast meal containing white bread, margarine, strawberry jam, cheese and tea, with or without milk, was digested in; vitro and the dialysis of copper investigated. Reference breakfasts were prepared with water and water with milk. Copper dialysability (percentage of copper dialysed) was markedly increased by tea. The influence of adding milk to tea was not significant. A second study was carried out in rats using the breakfasts containing tea and water. They were spiked with 64Cu and given orally to the animals in a single dose. Whole‐body 64Cu retention was measured over a 4‐day period after administration. In order to determine the true absorption of 64Cu a control group of rats received the dose intraperitioneally. The results in vivo indicate a tendency to higher absorption and retention of 64Cu with tea. Liver 64Cu retention was significantly elevated by tea. Therefore, the presence of tea in a breakfast meal favours the formation of soluble low‐molecular‐weight ligands which can be absorbed and retained by rats. The concomitant increase in liver copper retetition may be associated with higher bioavailability and/or lower copper mobilisation

    Introducción: La exposición a agentes biológicos es el riesgo más importante entre los estudiantes del título de enfermería, durante los procedimientos que implican contacto directo con fluidos corporales. Los pinchazos son los accidentes biológicos de mayor frecuencia, siendo los estudiantes especialmente susceptibles a estos durante las prácticas. Por ello, se hace imprescindible la formación en prevención entre estos alumnos. Numerosos estudios han demostrado como el uso de herramientas informáticas conducen a una mejora en el aprendizaje. Con el fin de minimizar los riesgos que afrontan los estudiantes de enfermería en sus prácticas, se ha realizado una investigación cuyos objetivos son: determinar los conocimientos previos, estimar la incidencia de accidentes por exposición a agentes biológicos; así como la creación e implementación de una web móvil formativa. Material y método: La metodología se ha estructurado en: 1º Realización de un estudio observacional descriptivo trasversal sobre una muestra de estudiantes de la Universidad de Córdoba, matriculados en el grado de enferme-ría y que han superado los practicums. 2º Desarrollo de una web formativa: (http://www.uco.es/investiga/grupos/LVRiesgosLaborales/formacion-sanitaria/ animaciones) Resultados: 1º Se han analizado los conocimientos básicos y específicos de los encuestados. 2º La incidencia de accidentes biológicos encontrada ha sido del 15,6%, de los que los pinchazos representan un 67%. 3º Se ha implementado un aula virtual funcional incluida en la web móvil formativa. Conclusiones: La valoración de conocimientos básicos establece que la frecuencia de respuestas correctas aumenta respecto a la superación de cursos en el grado. La incidencia de accidentes biológicos, aunque es baja, se incrementa al superar el número de practicums. El aula virtual desarrollada proporciona información, de manera interactiva, con el fin de disminuir la incidencia de accidentes biológicas, en especial los causados por vía parenteral
